Job summary

Lindley Medical Practice, part of gtd healthcare, is seeking an Advanced Clinical Practitioner to join a multidisciplinary team in Oldham. The role offers a competitive salary (£58,991–£71,281 per annum), pro rata with full NHS Pension, and opportunities to develop clinical leadership and quality improvement.

You will work with on-site salaried GPs, nurses, pharmacists, and admin staff, deliver care via telephone, video or face-to-face, and access the gtd healthcare learning academy and CPD time.

Qualifications

• Registered professional (nurse, paramedic, pharmacist or other AHP) with MSc Advanced Clinical Practice or equivalent
• V300 Non-Medical Prescriber registration and governance requirements
• Mentor/Teaching qualification or equivalent clinical supervision experience

Responsibilities

• Assess and holistically examine patients with minor and complex needs
• Plan, implement and evaluate management plans with safe discharge/referral
• Provide information on medications, side-effects and interactions
• Prioritise health problems and intervene in urgent or emergency situations
• Triage referrals and intake from external clinicians and patients
• Support wider team and promote quality improvement and learning
