Job summary

gtd healthcare is seeking an Advanced Clinical Practitioner to join Ashton GP Service in Ashton-under-Lyne. The role focuses on delivering safe, patient-centered care within a supportive multidisciplinary team, across two sites.

You will manage acute and routine presentations, perform assessments and diagnostics, prescribe independently, and support continuity for patients with long-term conditions while collaborating with GPs, nurses and the wider MDT.

Qualifications

• MSc in Advanced Clinical Practice or equivalent.
• Independently prescribes within scope of practice as ACP.
• Strong collaboration within multidisciplinary teams.

We are seeking a confident, autonomous Advanced Clinical Practitioner to work at Ashton GP Service. You will play a key role in delivering safe, effective and patient centred care within a supportive multidisciplinary team. Your responsibilities will include:

Advanced Clinical Practitioner £57,625 to £71,281 per annum, pro rata, depending on experience, yearly salary increments, NHS Pension provider. An exciting opportunity to work in a supportive, community focused and a well establishedpracticefor an Advanced Clinical Practitioner at Ashton GP Service. You will be supported by strong clinical leadership and a collaborative multidisciplinary team. The hours for this role are 30 hours per week, to be worked on Wednesday and Friday between 08:00 and 18:30.
