Job summary

gtd healthcare is seeking an Advanced Clinical Practitioner to join Ashton GP Service in Ashton-under-Lyne. The role focuses on delivering safe, patient-centered care within a supportive multidisciplinary team, across two sites.

You will manage acute and routine presentations, perform assessments and diagnostics, prescribe independently, and support continuity for patients with long-term conditions while collaborating with GPs, nurses and the wider MDT.

Qualifications

• MSc in Advanced Clinical Practice or equivalent.
• Independently prescribes within scope of practice as ACP.
• Strong collaboration within multidisciplinary teams.

Responsibilities

• Managing acute and routine presentations across both sites.
• Undertaking clinical assessments and diagnostics.
• Prescribing independently.
• Supporting continuity of care for patients with long-term conditions.
• Working collaboratively with GPs, nurses and the wider MDT.
• Contributing to service development and quality improvement.
