Job summary

East Durham College invites applications for two Transitions Officers, based at Houghall Campus and Willerby Grove. The role supports SEND/high needs learners transitioning from school to college and from college to employment or further progression.

You will liaise with schools, partners and college teams to ensure timely, well-informed transitions before enrolment. Essential: GCSE English and maths at standard level; Level 3 qualification or equivalent in education, SEND, health and social

Qualifications

• GCSE level standard education or equivalent (maths & English).
• Level 3 qualification or equivalent experience in education, SEND, health and social care, youth work, careers guidance, community support, or a related field.

Responsibilities

• Support the successful transition of learners with SEND and/or high needs from school into college and college into employment or alternative progression pathways.
• Act as first point of contact for schools and as a key link between external partners and internal college teams.
• Ensure accurate information about learner needs is shared in a timely way to enable appropriate support and planning.
