Job summary

NHS Humber and North Yorkshire Integrated Care Board is recruiting a Band 6 Complex Case Manager to oversee Section 117 aftercare across Hull, East Riding and North Lincolnshire. Home working is supported and you will work with a multidisciplinary team to ensure safe, appropriate aftercare and effective care planning.

The role requires skilled assessment, case management and collaboration with local authorities, clinicians and carers.

Qualifications

• Educated to degree level in relevant subject or equivalent experience
• Nurse registration RMN/RNLD or Social Worker qualification
• Relevant postgraduate qualification
• Demonstrate understanding of MHA section 117/MCA, capacity and consent
• Safeguarding principles and processes

Responsibilities

• Case manage patients subject to s117 aftercare and transforming care
• Ensure placements provide appropriate, good quality services and value for money
• Liaise with Legal team regarding DoLs/CoP enquiries
• Re-assess new patient placements within 12 weeks to ensure suitability
• Handle highly complex, sensitive information with a calm, balanced approach
• Attend reviews and CTR pre-discharge meetings to ascertain placement appropriateness
• Attend Mental Health Review Tribunals as required to provide update reports
