The job involves providing expert clinical leadership in NHS Continuing Healthcare, helping to implement the National Framework across fast track assessments, care planning and reviews. You will manage complex pathways, support governance, and work with senior leaders to improve patient outcomes and policy implementation.

Flexible working and occasional travel are available; enhanced DBS and right-to-work in the UK are required, with visa sponsorship not offered.

Qualifications

• Degree in a relevant subject or equivalent experience.
• Current registration with an appropriate regulatory body (NMC, AHP, midwife or social work).
• Significant experience in a politically sensitive environment.
• Experience coordinating projects in complex environments.
• Experience developing and implementing policy and SOPs.

Responsabilités

• Provide specialist clinical expertise across CHC, Funded Nursing Care and complex commissioning.
• Lead implementation of the National Framework for NHS Continuing Healthcare across Fast Track assessments and reviews.
• Manage complex pathways and provide senior clinical leadership.
• Contribute to service performance, quality governance and patient outcomes with the Senior Leadership Team.
• Manage clinical and operational risks and ensure accurate reporting.
• Support policy development and service improvement via data analysis.
• Contribute to resource, business and financial planning.
• Induct and train staff; lead multidisciplinary collaboration.
• Work autonomously with professional judgment and confidentiality.
• Occasional travel as required.
