St Mungo’s in London seeks an Assessment and Reconnection Worker to support people experiencing homelessness. You will provide personalised case management, turning assessments into practical action plans, and collaborate with a busy team to help clients on their recovery journey.

The role involves building relationships, understanding welfare and housing legislation, and working a 24/7 rota including weekends and nights in mixed shifts.

• Patience and resilience in challenging situations.
• Ability to adapt to new situations and learn.

Responsabilités

• Provide personalised case management to clients, turning assessments into action plans.
• Build and maintain relationships to support clients’ recovery.
• Develop knowledge of welfare benefit and housing legislation.
• Work across a 24/7 rota including weekends and nights.
• Collaborate within a busy team to assist people new to or at risk of rough sleeping.
