Job summary

Civic Recruitment Limited in West Midlands is seeking a dedicated Rough Sleeper Support Worker to provide outreach, support and intervention for vulnerable adults who are homeless or at risk. You will conduct borough outreach, develop care plans, assist with housing access and work flexibly to support clients’ independence.

You will work to safeguard, manage a varied caseload and coordinate with services to improve wellbeing, with a bi-weekly pay schedule and a requirement for a full UK driving

Qualifications

• Experience supporting homeless individuals or vulnerable adults.
• Experience with support planning, risk and needs assessments.
• Knowledge of homelessness services and housing support.
• Understanding of safeguarding practices.
• Experience with individuals with complex needs.
• Knowledge of substance misuse, mental health and offending behaviours.
• Strong case management and record-keeping.
• Ability to work independently and manage a varied caseload.
• Excellent communication and engagement skills.
• Full UK Driving Licence and access to a vehicle.

Responsibilities

• Conduct outreach activities across the Walsall borough to identify and engage rough sleepers.
• Follow up referrals and undertake proactive casework interventions.
• Complete holistic risk and needs assessments.
• Develop and review support plans and risk management plans.
• Support service users to access accommodation and sustain tenancies.
• Encourage engagement with education, training and employment opportunities.
• Support service users to build family and community support networks.
• Assist with benefit claims and income maximisation.
• Accompany service users to appointments when required.
• Respond to crises and provide practical and emotional support.
• Maintain accurate case records and support documentation.
• Safeguard vulnerable adults and escalation concerns appropriately.
• Support volunteers, trainees and new team members where required.
