Job summary

Connection Support in the United Kingdom is seeking two Rough Sleeper Case Workers to support clients in High Wycombe and the surrounding areas, and in Aylesbury and the surrounding areas. You will work in a person-centred way to help clients sustain accommodation, build confidence, and develop skills for lasting independence.

Qualifications

• Experience of working within housing/homelessness or a related support environment.
• Understanding of the causes of homelessness and challenges moving from rough sleeping into settled accommodation.
• Ability to build positive, professional relationships and motivate people to make positive changes.
• Experience of supporting people with a range of support needs and helping them access services within their community.
• Good organisational, record-keeping, and basic IT skills, including Outlook and Microsoft Office applications.

Responsibilities

• Support clients to sustain their accommodation and prevent a return to rough sleeping.
• Work with clients to identify goals, develop support plans, and achieve positive outcomes.
• Help people access housing, welfare benefits, healthcare, education, training, and employment opportunities.
• Support clients to improve their financial stability and develop practical life skills.
• Work collaboratively with local authorities, housing providers, landlords, and partner agencies to provide coordinated support.
• Provide occasional support to the Rough Sleeper Outreach Team, including verifications during periods of annual leave and absence.
