Job summary

Carrington West is recruiting for a Rough Sleep Support Worker to provide outreach across Northamptonshire, engaging people sleeping rough and guiding them toward accommodation and services. You’ll conduct needs assessments and create tailored support plans within a frontline, collaborative setting.

This role involves complex caseloads, safeguarding responsibilities, and working outside standard hours to achieve sustainable outcomes for vulnerable individuals.

Qualifications

• Experience in housing, homelessness, care or related support environment.
• Experience with assessments, support planning and advocacy.
• Knowledge of homelessness issues and barriers to accommodation.
• Ability to manage challenging situations with complex needs.

Responsibilities

• Deliver flexible street outreach and engage people sleeping rough to access accommodation and support.
• Complete risk and needs assessments and develop support plans.
• Provide intensive, person-centred support for individuals with complex needs.
• Work with Housing Options, Adult Social Care, health services and other partners.
• Manage safeguarding concerns and maintain accurate case records.
• Support people moving into temporary and longer-term accommodation and sustain tenancy.
