Job summary

4Recruitment Services are seeking a Rough Sleeper Outreach Worker to work for our client across the North Northamptonshire area. A valid UK driving licence and access to a vehicle are essential for this role, as the position requires on-site working 5 days per week.

You will deliver an intelligence-led street outreach service, engage with rough sleepers, develop personalised housing plans, and work with Housing Options and other partners to prevent repeat homelessness.

Qualifications

• Experience in housing or care settings providing support to vulnerable customers.
• Practical experience with assessment, providing support, key-working and advocacy.

Responsibilities

• Deliver a street outreach approach targeted at rough sleepers’ hotspots.
• Engage and build trust with rough sleepers to provide housing and support.
• Develop Personalised Housing Plans in partnership with Housing Options.
• Coordinate with Adult Social Care and other partners for safeguarding and multi-agency meetings.
• Collaborate with hospitals, prisons and landlords to prevent homelessness.
• Provide housing-related support to those in temporary accommodation.
