Job summary

NHS Jobs is seeking a Band 5 Rotational Occupational Therapist to perform autonomous assessments, develop evidence-based intervention plans, and work across community settings. You will review treatment plans, manage clinical risk, adapt equipment and seating assessments, and ensure safe discharge planning in collaboration with a multidisciplinary team.

The role requires engagement with service users, families, and carers, maintaining HCPC-aligned documentation, and supporting rehab assistants

Responsibilities

• Conduct autonomous assessments using the Occupational Therapy Process and develop evidence-based intervention plans.
• Work autonomously in community settings, assessing and managing risk.
• Review and adapt treatment plans to ensure progress and safe discharge planning.
• Manage clinical risk, ensuring safe, appropriate and timely interventions.
• Prescribe, deliver, fit, and adjust equipment and minor/major adaptations to enhance independence and safety.
• Complete screening of cognitive impairments under supervision and escalate concerns as appropriate.
• Complete seating assessments under supervision and prescribe appropriate measures.
• Engage and motivate service users, families, and carers in rehabilitation.
• Manage emotionally challenging situations with empathy and professionalism.
• Maintain accurate legal and professional documentation in line with HCPC and Bromley Healthcare standards.
• Work effectively within a multi-disciplinary team, representing the Occupational Therapy perspective.
• Build strong relationships with health, social care, private, and voluntary sector professionals.
• Communicate effectively with referrers and stakeholders, ensuring clear care coordination and patient-centered delivery of care.
• Support and supervise rehab assistants and therapy students, delegating appropriately.
• Actively participate in team meetings and service development initiatives.
