Job summary

Wye Valley NHS Trust is seeking a Band 6 Therapy Clinical Decision Maker for the Single Point of Access, delivering telephone-based triage and assessment of therapy needs. The post involves 2 days, 8-6, Monday to Friday, based at the Community Referral Hub in Hereford.

You will provide clinical assessment, triage, and autonomous decisions to ensure timely access to community care, working with a MDT and across partners to minimise deterioration and hospital admissions.

Qualifications

• Registered healthcare professional with HCPC registration.
• Degree level qualification or equivalent.
• Post graduate experience in health, social care and community services.
• Experience with remote assessment and triage.
• Strong communications and organisational skills.

Responsibilities

• Undertake comprehensive telephone-based clinical triage and assessment of patients with therapy needs.
• Complete triage and screening of urgent referrals, directing to the most appropriate service.
• Collaborate with MDT members to determine urgency, risk and appropriate action.
• Recognise signs of acute deterioration and safeguarding concerns and take action.
• Coordinate pathways with community teams, primary care and social care partners.
• Contribute to clinical pathways, triage frameworks and service guidance.
• Participate in clinical governance activities and quality improvement.
