Qualifications

• Advanced clinical assessment, triage and decision making.
• Good and comprehensive awareness of community services and networking ability.
• Competence in using electronic patient records and IT skills.
• Able to undertake audit and write reports.

Responsibilities

• Undertake comprehensive, predominantly telephone-based clinical triage and assessment of patients presenting with acute, complex health and care needs.
• Work autonomously as a senior clinical decision-maker to determine urgency and the most appropriate course of action.
• Analyse and interpret complex information to formulate clinical decisions and management plans.
• Recognise signs of deterioration and safeguarding concerns, taking immediate action when required.
• Support management of patients with complex presentations and escalating needs, coordinating multidisciplinary intervention.
