Job summary

Birmingham Community Healthcare NHS Foundation Trust is recruiting a Bank Band 6 Single Point of Access Clinician to join the Priestley Wharf service. You will provide expert clinical triage and assessment for patients with acute and long-term conditions, supporting safe discharge and reducing avoidable hospital admissions.

You will work autonomously within agreed pathways, collaborating with GPs, hospital clinicians and social care partners, and rotate across sites to cover a seven-day service

Qualifications

• Current UK professional registration is required.
• Driving Licence and access to a vehicle are essential.

Responsibilities

• Delivering safe, evidence-based clinical assessment and triage.
• Recognising deterioration and red-flag symptoms and taking appropriate action.
• Coordinating referrals and access to community, medical and diagnostic services.
• Communicating effectively with patients, carers, families and professionals.
• Maintaining confidentiality and complying with information governance.
