Bristol NHS Foundation Trust is seeking a dedicated Patient Pathway Assistant for the Weight Management Department (WAMS). You will oversee referrals, coordinate assessments, and ensure follow-up care across a multidisciplinary team, aligning with government targets and local access policies.

The role involves handling referrals, booking appointments, and uploading documentation to Bluespier and Careflow, while maintaining patient safety and service quality in a fast-paced NHS environment.

Qualifikationen

• Minimum five GCSE/O’level qualifications including English Language and Mathematics.
• NVQ Level 3 in Business Administration (Desirable).
• Relevant health or social care experience at AFC Band 2, 3 or equivalent.

Aufgaben

• Answer patient queries via telephone and email.
• Booking clinical appointments using PAS systems (Careflow and Bluespier).
• Problem-solving proactively when managing patient flow (DNAs, cancellations, waiting list transfers).
• Liaising with clinicians to action outcomes for patients on the pathway.
• Ensuring timeliness of clinic bookings to tackle long waiting lists.
• Valuing constant and continuous communication within the team via email and in-person.
• Caring in communications to provide compassionate, patient-centred service.
