Job summary

Qualifications

• Registered Nurse with valid NMC pin and UK work eligibility.
• Experience in complex care and/or community nursing.
• Strong clinical knowledge and sound professional judgement.
• Excellent organisational and time-management skills.
• Ability to manage a caseload and prioritise competing demands.
• Excellent verbal and written communication skills.
• A calm, caring and professional approach.
• Demonstrated leadership and ability to support teams.
• Full UK driving licence and access to a car.
• Willingness to travel across a wide geographical area.
• Right to live and work in the UK.

Responsibilities

• Managing a small caseload of complex care clients and their care teams.
• Providing clinical leadership and oversight to care teams.
• Building strong relationships with clients, families, commissioners and case managers.
• Regularly reviewing care packages to ensure they remain safe, effective and person-centred.
• Supporting, mentoring and developing care and nursing staff.
• Ensuring staff feel valued, supported and engaged.
• Providing on-call support as part of a four-week rotational on-call system.
• Ensuring care plans, risk assessments and clinical documentation remain accurate and compliant with CQC requirements.
