Job summary

Practice Plus Group, in the Health in Justice service, is seeking a dedicated role within the Reconnect team in North Yorkshire. The position focuses on supporting people leaving prison within 12 weeks, linking health and social care with police, probation, and courts.

Regional travel is involved, and right to work in the UK is required with vetting and security checks. You will work to reduce reoffending by addressing health and social needs, liaising with multi-agency teams, and providing

Qualifications

• Face-to-face work with service users who have a variety of health needs
• Experience of working within health and/or social care settings
• Ability to collaborate with multiple agencies and professionals

Responsibilities

• Support children and adults through the criminal justice process
• Assist access to health needs including mental health, substance misuse, homelessness, and safeguarding
• Coordinate with police, probation, courts, and social care services to ensure continuity of care
• Make onward referrals to ensure smooth transitions and reduce reoffending through health interventions
