Job summary

Practice Plus Group is seeking a Reconnect Support Time & Recovery Navigator for a part-time role (17.5 hours per week). You will join the Reconnect service and work across a wide network, helping adults and children engage with the Criminal Justice System and access health and social care support in the North Yorkshire region.

Location can be Scarborough, Harrogate or York, with a focus on early intervention to reduce reoffending.

Qualifications

• Experience working with service users face to face.
• Understanding of multi-disciplinary and multi-agency working.
• Non-judgmental and compassionate approach.
• Person-centred model focused on meaningful pathways and early intervention.

Responsibilities

• Liaise with police, probation, courts and legal services as well as inpatient and community health and social care services.
• Support access to health needs including mental and physical health, substance misuse, homelessness and safeguarding.
• Assist service users leaving prison within 12 weeks with onward referrals to ensure continuity of care.
