Job summary

Forward Trust is seeking a RECONNECT Practitioner to support people leaving prison or immigration detention to access healthcare and community services. Based in Bedfordshire, you will assess needs, develop care plans and connect clients with local health and wellbeing services, with safeguarding and person-centred practice at the core.

This role requires a full UK driving licence, empathy, strong communication, and the ability to work with partners across prison, healthcare and community

Qualifications

• Full UK driving licence and access to own vehicle.
• Compassionate, empathetic and non-judgemental approach.
• Excellent communication and interpersonal skills with diverse people.
• Ability to identify needs and provide tailored advice and support.
• Strong organisational skills to manage referrals and records.

Responsibilities

• Manage referrals and assess suitability with timely updates.
• Assess physical and mental health and wellbeing needs.
• Provide ongoing support before release and up to six months post-release.
• Develop personalised care plans with partners.
• Connect service users to GP referrals, healthcare and aftercare.
• Collaborate with prison teams, healthcare professionals and partners.
• Support safe transitions and uphold safeguarding standards.
