Job summary

Lancashire & South Cumbria NHS Foundation Trust is seeking a ReConnect Practitioner to support prisoners in East Lancashire and across Lancashire & Cumbria. The role covers pre-release triage, assessment and post-release support up to six months, with a focus on ensuring access to healthcare and reducing health inequalities.

Qualifications

• Degree qualification in relevant subject or demonstrable evidence.
• Desirable: RMN, Learning Disability Nurse, Social Worker, Occupational Therapist.
• Desirable: Psychology Degree.

Responsibilities

• Vetting and DBS checks for staff entering prison/probation estate.
• Ensure Enhanced DBS status and compliance.
• Maintain timely and accurate documentation on electronic records.
• Lone working as required.
