Job summary

Lancashire and South Cumbria NHS Foundation Trust is seeking a Reconnect Practitioner to bridge prison and community healthcare across East Lancashire and beyond. You will assess needs, coordinate care and support timely transitions for individuals leaving custody.

You will work with prisoners nearing release, ensuring safeguarding, DBS checks and comprehensive documentation in electronic records, with a focus on reducing health inequalities and continuity of care in the community.

Qualifications

• Degree qualification in relevant subject or demonstrable evidence or equivalent experience relevant to field of practice.
• Professional recognised qualification such as RMN, Learning Disability Nurse, Social Worker, Occupational Therapist.
• Knowledge of mental health issues and/or learning disability.
• Experience with working with people with complex needs for a minimum of 2 years.

Responsibilities

• Reconnect will create an effective link between prison & community in order to increase access & uptake of healthcare or relevant support services.
• Identify and respond to requests to screen and assess people with vulnerabilities for persons approaching release.
• Provide support within the community upon release from prison.
• Ensure DBS status and clearances are completed as part of vetting and safeguarding processes.
