Job summary

Forward Trust is seeking a dedicated RECONNECT Practitioner to aid individuals leaving prison or immigration detention in accessing healthcare, wellbeing support and community services. You will assess needs, create bespoke care plans, and coordinate with healthcare, community organisations and prison teams to ensure seamless transitions.

Qualifications

• A full UK driving licence and access to own vehicle are essential.
• Compassionate, empathetic and non-judgemental approach to supporting people with complex needs.
• Excellent communication and interpersonal skills with diverse groups.
• Ability to identify individual needs and provide tailored advice and practical support.
• Strong organisational skills for managing referrals and records.

Responsibilities

• Manage referrals and provide timely updates to referrers.
• Assess service users’ physical health, mental health and wellbeing needs.
• Provide ongoing support before release and up to six months post-release.
• Develop personalised care plans with service users and partners.
• Signpost to GP referrals, healthcare access and local services.
• Build and maintain partnerships with prison staff and external agencies.
• Coordinate referrals across prisons, healthcare and families.
