Job summary

Via is seeking a Recovery Practitioner to deliver substance misuse treatment and recovery support across Cheshire West & Chester. This community-based role involves daily travel to service hubs, rural communities, and people’s homes.

You will undertake assessments, home visits and outreach, manage a caseload, and deliver harm reduction, psychosocial interventions and overdose prevention advice. A trainee position (24 hours per week) is available.

Responsibilities

• Undertake assessments, home visits and outreach across Cheshire West & Chester.
• Manage a caseload to provide timely harm reduction and psychosocial support.
• Deliver overdose prevention advice and connect people with health and wellbeing services.
• Collaborate with multi-disciplinary teams and uphold safeguarding principles.
• Support joined-up care alongside Via Health and Wellbeing Coach to improve access to services.
