Job summary

Via is seeking a Recovery Practitioner to deliver substance misuse treatment and recovery support across Cheshire West & Chester. This community-based role involves daily travel to service hubs, rural communities, and people’s homes.

You will undertake assessments, home visits and outreach, manage a caseload, and deliver harm reduction, psychosocial interventions and overdose prevention advice. A trainee position (24 hours per week) is available.

We’re looking for Recovery Practitioner for our Cheshire West and Chester Service.

Within this role, you will play a vital part in delivering substance misuse treatment and recovery support for individuals across Cheshire West & Chester. This is a community-based role which requires daily travel across the borough, including regular travel to rural communities, service hubs, satellite locations and people’s homes.

The successful candidate will undertake assessments, home visits and outreach appointments while travelling across Cheshire West and Chester. You will manage a caseload, ensuring timely and effective support for service users. You will deliver harm reduction advice, psychosocial interventions, and overdose prevention advice.

Work collaboratively with colleagues and key stakeholders across multi-disciplinary teams, ensuring adherence with all Via policies and safeguarding principles. The post holder will also work closely with Via’s Health and Wellbeing Coach, who works across the county, to support joined-up care and improve access to health and wellbeing support for people using the service.
