Job summary

Waythrough invites applications for a Recovery Coordinator to join the Kirkgate Active Team at Armly Court. You will support adults in treatment for opiate use, build therapeutic relationships, manage your own caseload and work with partner agencies to provide coordinated, person-centred care.

You will undertake triage, assessments and tailor interventions, help access healthcare, housing and employment, and maintain records using SystmOne.

Qualifications

• Experience managing a caseload in health, social care or substance use services.
• Knowledge of supporting people affected by alcohol/drug use, especially opiate dependence.
• Strong communication and relationship-building skills.
• Ability to work independently within a multidisciplinary team.
• Health and Social Care qualification or equivalent experience beneficial.

Responsibilities

• Manage a caseload of adults in treatment for opiate use and develop recovery plans.
• Conduct triage, screening and comprehensive assessments, tailoring interventions to individual needs and goals.
• Build therapeutic relationships to encourage engagement and promote recovery.
• Coordinate care with prescribing clinicians and partner agencies.
• Help access healthcare, housing, employment, family support and community resources.
• Maintain accurate records using SystmOne.
• Contribute to a positive team delivering person-centred care.
