Job summary

Change Grow Live is recruiting for Opiate Team Recovery Coordinators in Croydon. You will work within a multidisciplinary team to support individuals on their recovery journey, building trust and delivering person-centred, evidence-based interventions.

You will collaborate with clinical colleagues, social care, housing providers and community partners to coordinate care, promote safer outcomes and help people move towards independent, healthier futures.

Qualifications

• Experience with people facing multiple and complex challenges.
• Understanding of recovery-focused approaches and harm reduction.
• Strong communication and relationship-building skills.
• Confidence engaging with people from diverse backgrounds and experiences.
• Knowledge of safeguarding, risk management and professional boundaries.
• Ability to manage competing priorities and maintain high-quality case management.
• Commitment to equality, inclusion and empowering others.

Responsibilities

• Build relationships with individuals accessing the service and tailor support to their needs.
• Support people to identify opportunities and overcome barriers to recovery.
• Assist with assessment, recovery planning and evidence-based interventions.
• Collaborate with clinical colleagues, housing providers and community partners to deliver coordinated support.
• Support individuals to engage with treatment, improve health, and pursue education or employment.
