Job summary

Change Grow Live is seeking Recovery Coordinators to support individuals in the opiate team in Stockton-on-Tees. This part-time role (22.5 hours a week) offers the chance to build meaningful relationships, conduct assessments, and deliver psychosocial interventions alongside prescribing clinicians.

You will help people on their recovery journey, promote access to housing, education and healthcare, and work closely with GPs, social care, and probation to coordinate care. DBS check will apply.

Qualifications

• Excellent communication and interpersonal skills.
• Compassionate, non-judgemental, and person-centred approach.
• Experience in health, social care, mental health, criminal justice, or substance misuse services is desirable.

Responsibilities

• Manage a caseload of individuals accessing structured treatment for opiate dependence.
• Complete comprehensive assessments, risk assessments, and recovery plans tailored to individual needs.
• Deliver evidence-based psychosocial interventions, motivational interviewing, and harm reduction advice.
• Support individuals prescribed opioid substitution treatment (OST), ensuring regular reviews and collaboration with prescribing clinicians.
• Promote recovery capital by supporting access to education, employment, housing, healthcare, and community resources.
• Facilitate regular key working sessions, monitor progress, and adapt recovery plans as needed.
• Carry out safeguarding responsibilities and escalate concerns per procedures.
• Encourage engagement with peer support and recovery communities.
• Collaborate with GPs, mental health services, probation, housing, social care, and partner agencies to deliver coordinated care.
• Maintain confidential electronic case records per policy and data protection laws.
• Participate in multidisciplinary meetings, case reviews, and clinical supervision.
• Support individuals through planned treatment completion, aftercare, and sustained recovery.
