Job summary

Waythrough is seeking a Recovery Coordinator to support adults in treatment for opiate use at the Armley Court site in Leeds. You will manage your own caseload within a collaborative, multidisciplinary team, delivering person-centred care and building meaningful therapeutic relationships.

Based at Armly Court, you’ll work with partners to coordinate care, promote recovery, and help clients access healthcare, housing and employment opportunities.

Qualifications

• Experience managing a caseload within health, social care or substance use services.
• Strong communication and relationship-building skills are essential.
• Ability to work independently within a multidisciplinary team.

Responsibilities

• Manage a caseload of adults receiving treatment for opiate use, developing, implementing and reviewing personalised recovery plans.
• Complete triage, screening and comprehensive assessments, tailoring interventions to individual needs and goals.
• Build trusting, therapeutic relationships that encourage engagement, reduce harm and promote sustained recovery.
• Work collaboratively with prescribing clinicians and partner agencies to coordinate care and provide integrated support.
• Help people access healthcare, housing, employment, family support and community resources to strengthen their recovery.
• Maintain accurate records of recovery plans, safeguarding, risk management and clinical interventions using SystmOne.
• Contribute to a positive and supportive team while delivering high-quality, person-centred care.
