The Role

We are seeking a Recovery Coordinator to support service delivery within Reconnected to Health at HMYOI Deerbolt. The successful candidate will provide one-on-one support to service users, from brief interventions to structured treatment, including group work. The approach is holistic, focusing on all aspects of wellbeing to achieve optimal recovery outcomes.

1. Manage a caseload, developing, implementing, and reviewing recovery plans in collaboration with the service team, including family and significant others where appropriate.
2. Conduct triage and comprehensive assessments with service users.
3. Work with partner agencies to undertake shared assessments, key working, and interventions to facilitate progression from engagement to self-sustained recovery.
4. Assist service users in accessing treatment and community resources to enhance recovery and social capital, supporting families within the community.
5. Lead case management activities, including risk assessments, safety planning, and safeguarding, utilizing online case management systems effectively.
