Recovery Coordinator

Location: HMP Deerbolt, Barnard Castle, DL12 9BG

Salary: £24,242 - £30,790 per annum

We address poverty and disadvantage through services in mental health, substance misuse, housing, and related support.

We operate nearly 200 services across England with close to 3,500 staff and volunteers supporting around 125,000 individuals annually.

The Role

We are seeking a Recovery Coordinator to support delivery within Reconnected to Health at HMYOI Deerbolt. You will provide one-to-one support, from brief interventions to structured treatment, including group work, tailored to individual recovery journeys. A holistic approach will be essential to encompass all aspects of wellbeing, aiming for optimal outcomes.

Responsibilities include:

1. Managing a caseload, developing, implementing, and reviewing recovery plans in collaboration with the service team, family, and significant others as appropriate.
2. Conducting triage, screening, and comprehensive assessments with service users.
3. Collaborating with partner agencies for shared assessments, key working, and interventions to facilitate progression from engagement to self-sustained recovery.
4. Facilitating access to treatment and community resources, supporting social capital and family involvement.
5. Leading case management activities, including risk assessments, risk management, safeguarding, and maintaining an online case management system.
