Waythrough is a major mental health and social support charity in England. This role is based at the Centre for Change in Durham, offering person-centred support from initial assessment through to treatment and sustained recovery.

The role emphasizes collaboration with service users, families and partner agencies to reduce harms from substance use and promote independence. You will work 37 hours per week, with a Monday–Friday pattern and occasional Saturday mornings.

Qualifications

• NVQ Level 3 in Health & Social Care or equivalent, or willingness to work towards it.
• Strong communication, organisational and IT skills.
• Empathetic, non-judgemental and person-centred approach.

Responsabilités

• Manage caseload and develop, deliver and review individual recovery plans.
• Complete triage, screening and comprehensive assessments.
• Carry out risk assessments and safeguarding plans.
