Job summary

Home Group Limited is recruiting for a Support Coordinator in Scunthorpe. The role involves leading support planning, coordinating Support Workers to deliver person-centred care, and balancing safeguarding, administrative duties, and meetings while guiding colleagues and managing caseloads.

You will help customers with daily tasks such as budgeting, cooking and appointments, working with multi-agency teams to safeguard and empower individuals, and preparing them for a move-on into the community.

Qualifications

• Passion and experience in supporting others ideally in a supported housing setting.
• Level 3 Diploma in Care or equivalent experience (or are willing to work towards it).
• Experience assessing referrals and coordinating support.
• The ability to work under your own initiative, remain calm under pressure and have a resilient approach.

The practical bits

• The role operates on a five-week rolling rota and includes a variety of shifts patterns, such as early shifts starting at 7am, late shifts finishing at 11pm, mid shifts, waking nights, and ocassional sleep-in shifts when required.
• The role requires regular travel across the East Yorkshire region, including services in Scunthorpe and Hull.
• You need an Enhanced with barring list (we pay)
• A vehicle insured for business use (and a license!), we’ll pay for any business mileage
