The Place Care is recruiting an experienced Care Coordinator – Complex Care to join our team in London.

The role includes coordinating complex care packages, managing rotas and shift cover, supporting care plans and risk assessments, monitoring care quality and documentation, and liaising with service users, families, care staff and health and social care professionals.

The post holder will also support safeguarding, incident management, service reviews, staff spot checks and new care package mobilisation.

Previous complex care experience is essential. A full UK driving licence is required. A company vehicle can be provided for work-related travel.
