St Helens Borough Council is recruiting a Reablement Team Supervisor to support adults 18+ at home, delivering care and rehabilitation programmes and working in a multi-disciplinary team.

You will use electronic care systems, liaise with Health and Social Care professionals, and support timely hospital discharge through the Home First collaboration with Whiston Hospital.

The post offers training, a driving license, and a 28-hour weekly rota across early and late shifts, based at Nightingale House.

Qualifications

• Experience with reablement and care rehabilitation programmes.
• Experience with electronic care systems.
• Flexible and able to use initiative when putting care plans into practice.
• A driving license and access to a vehicle is essential.

Responsabilités

• Lead and supervise reablement care delivery for adults 18+, ensuring independence and safety.
• Collaborate with Health and Social Care professionals to coordinate services.
• Utilise electronic care systems to record care activity and outcomes.
