Job summary

Northern Care Alliance NHS Foundation Trust is seeking a Home First Reablement Support Worker to assist clients at home, enabling safe discharge and independence in Salford. You will work as part of the Home First Rehab & Reablement team, providing compassionate, person-centered care and clear communication with patients and families.

The role requires a full driving licence, willingness to work a rota (including weekends/Bank Holidays) and a commitment to high-quality, patient-focused care

Qualifications

• Full driving licence and access to own vehicle.
• Experience in home-based care and assisting daily living activities.
• Able to work on a rota including weekends and Bank Holidays.

Responsibilities

• Assist individuals with personal care tasks (washing, dressing, meals, medications).
• Assist in safe lifting, transferring and repositioning.
• Support individuals to live independently at home and achieve outcomes.
