Responsibilities

• Build positive relationships with customers and their family carers to promote health, wellbeing and independence.
• Support individuals through personalised care plans and short-term enablement strategies.
• Promote services that enable safe, independent living at home.
• Monitor progress against care plans and maintain accurate records for reviews.
• Travel to people's homes in rural locations and drive in varying conditions.
