Job summary

Wiltshire Council is seeking Reablement Support Workers to help individuals in their homes after hospital discharge. You will support customers to achieve goals, working closely with occupational therapists to co-develop personalised reablement plans.

The role requires a valid UK driving licence and access to a vehicle, and you must be available on a rota basis between 7am and 10pm, 365 days a year. NVQ apprenticeships and ongoing upskilling are offered to develop your career.

Qualifications

• NVQ apprenticeships and upskilling opportunities are available.
• Experience in care or support work is beneficial.
• Good communication and teamwork required.

Responsibilities

• Provide in-home support post-hospital discharge to help customers meet goals.
• Collaborate with occupational therapists to develop personalised reablement plans.
• Travel to customer visits as required.
