As Reablement Social Work Practice Lead, you'll play a key role in supporting adults and carers to achieve positive outcomes, maximise independence and access the right support at the right time. Working within a multi-disciplinary Reablement and Independence Service, you'll provide professional leadership, guidance and supervision to Reablement Social Workers, while also holding a small caseload of more complex work.
