You will ensure person-centred and legally compliant practice to promote independence and smoother transitions. Working within a multi-disciplinary team, you will drive service improvements, support audits, and maintain accurate records while collaborating with therapy and locality teams.

Responsibilities

• Lead and supervise Reablement Social Workers, with a small caseload of complex cases.
• Undertake strengths-based assessments, care planning, reviews, risk assessments and capacity assessments.
• Improve social work practice, support audits, advise on complex cases, safeguard, and maintain high-quality records.
