Job summary

West Berkshire Council is seeking a Reablement Practitioner to join the Adult Social Care Service, delivering high-quality reablement support to residents in Newbury. You will assess needs, implement plans and monitor progress to promote independence.

The role involves working with service users, carers and professionals, with DBS clearance and continuous registration with the DBS Update Service required. A flexible pattern of hours is expected.

Job description

Are you passionate about supporting individuals to achieve their maximum potential and promote independence? West Berkshire Council is seeking a dedicated and motivated Reablement Practitioner to join our Adult Social Care Service, working within the Reablement Team. The Role As a Reablement Practitioner, you will play a crucial role in delivering high-quality reablement services to our residents. Reporting to the Team Manager - Promoting Independence (Reablement and Sensory Needs), you will be responsible for:

  • Working directly with individuals to assess their needs and provide tailored support to enable them to regain independence in daily living activities.
  • Implementing reablement plans and monitoring progress, ensuring effective communication and collaboration with service users, carers, and other professionals.
  • Conducting initial assessments and reviews, providing accurate and timely information to inform decision-making.
  • Advising and guiding individuals on how to overcome barriers to independence, promoting self‑care and empowering them to achieve their goals.
  • Working closely with other teams and agencies to ensure holistic care and support for individuals, including signposting to additional services as required., This role has been identified as public facing in accordance with Part 7 of the Immigration Act 2016; the requirement to fulfil all spoken aspects of the role with confidence in English applies.

This role involves regulated activity with adults at risk. As such, appointment is subject to an Enhanced DBS check with the relevant Barred List(s). In this post you will be required to maintain continuous registration with the DBS Update Service for the duration of your employment. Requirements
  • A solid understanding of reablement principles and practices, with experience in delivering direct support to individuals.
  • Excellent assessment and intervention skills, with the ability to identify and address barriers to independence.
  • Strong communication and interpersonal skills, enabling effective collaboration with service users, colleagues, and external partners.
  • Ability to work autonomously, managing a caseload and prioritising tasks effectively.
  • A positive and empowering approach, fostering motivation and self‑belief in individuals.

This role has a fixed working pattern: 7 working days in 14
32.5 hours per week - morning (07:00-10:30), lunchtime (12:00-13:30) and evening (18:00 - 22:00)
27.25 hours per week - morning (07:00-10:30), lunchtime (12:00-13:30) and teatime (16:00-18:30)
21 hours per week - PM 16:00-22:00
We are ideally looking to offer the successful candidate the full number of hours advertised above. We may however consider offering less hours than advertised providing they meet the requirements of the role and service which will be determined by the recruiting manager.
