Job summary

Slough Borough Council is recruiting a Reablement Social Work Practice Lead in Slough. The role involves providing professional leadership to Reablement Social Workers and managing a small complex caseload.

You will ensure person-centred and legally compliant practice to promote independence and smoother transitions. Working within a multi-disciplinary team, you will drive service improvements, support audits, and maintain accurate records while collaborating with therapy and locality teams.

  • Improve social work practice, support audits, advise on complex cases, safeguard, and maintain high-quality records.

  • Pay Level:8, £49, 868 - £55,273(inclusive of Local Weighting Allowance of £1,130 per annum)
  • Contract Type: Permanent
  • Working Hours:37hours per week
  • Location:Observatory House/ Slough Hybrid(minimum 2 days per week in the office)
  • DBS requirement:Enhanced

What you’ll be doing:

As Reablement Social Work Practice Lead, you’ll play a key role in supporting adults and carers to achieve positive outcomes, maximise independence and access the right support at the right time. Working within a multi-disciplinary Reablement and Independence Service, you’ll provide professional leadership, guidance and supervision to Reablement Social Workers, while also holding a small caseload of more complex work.

You’ll undertake strengths-based assessments, care and support planning, reviews, risk assessments, Mental Capacity and Best Interests assessments, ensuring practice is person‑centred, legally compliant and focused on promoting independence. You’ll work closely with therapy colleagues, locality teams, hospital social work teams and external partners to support joined‑up working and smooth transitions where longer‑term care needs are identified.

A key part of the role will be improving and developing social work practice within the service. You’ll support evidence‑based approaches, contribute to audits, provide advice on complex cases, support safeguarding activity where required, and ensure accurate records are maintained so residents and carers receive high‑quality, timely and effective support.
