Slough Borough Council invites applications for the role of Reablement Social Work Practice Lead. You will lead a multidisciplinary team, supervise Social Workers, and carry a small caseload of complex cases to promote independence and safe, person-centred care.

Qualifications

• Provide professional leadership, guidance and supervision to Reablement Social Workers.
• Undertake strengths-based assessments and care/support planning.
• Ensure legal compliance and person-centred practice.

Responsibilities

• Lead a Reablement Social Work team within a multidisciplinary service.
• Hold a small caseload of more complex work while supervising others.
• Promote evidence-based practice and contribute to audits.
