Job summary
Care Dorset Ltd in Dorchester is recruiting a Community Reablement Worker to provide domiciliary reablement support at clients’ homes, helping individuals regain independence and improve quality of life through person-centred care.
You will work with occupational therapists, nurses and social workers to assess needs, create care plans, monitor progress and ensure all activities meet CQC standards. Training and development opportunities are available.
Qualifications
- Experience of delivering domiciliary reablement or similar care in clients' homes.
- Ability to assess needs and develop person-centred care plans.
- Ability to work with multidisciplinary teams.
- Ability to monitor progress and maintain records.
Responsibilities
- Deliver reablement care in clients' homes to promote independence.
- Administer medication as directed by a GP per guidelines.
- Develop and implement personalised care plans.
- Conduct initial assessments and regular reviews.
