Job summary

Norfolk County Council's Caring for Better Outcomes (CFBO) invites applications for Reablement Assistant Practitioner to provide therapy-led reablement support across the county, helping people regain skills after hospital discharge.

This hybrid, field-based role requires strong communication, careful case recording and collaboration with therapists, managers and partner agencies to plan timely, person-centred outcomes.

Qualifications

• Good communication and interpersonal skills for engaging with people, families and professionals.
• Ability to document case notes accurately and maintain records in line with requirements.
• Ability to work collaboratively with therapists, managers and partner agencies.

Responsibilities

• Completing timely, proportionate and strengths-based Care Act assessments following reablement.
• Identifying eligible needs, agreeing person-centred outcomes and arranging sustainable support.
• Working with Assistant Practitioner Occupational Therapists to understand functional ability, progress and opportunities to maximise independence.
• Working with Operational Managers to progress cases, support timely decisions and avoid delays.
• Building relationships with locality teams to support coordinated planning and handovers.
• Communicating effectively with people, families, provider agencies and partner professionals.
• Maintaining accurate case records in line with statutory and organisational requirements.
• Managing a countywide caseload according to need, risk and urgency.
