Job summary

Norfolk Community Health and Care NHS Trust is seeking a Band 4 Assistant Practitioner for the Willow Therapy Unit. The role involves supporting inpatient reablement across a 48-bed unit and working across Occupational Therapy and Physiotherapy disciplines.

The post requires shift work across a 7-day service with Bank Holidays, and interviews will be conducted face to face only. This role emphasizes collaboration, independence, and a proactive reablement ethos within a multidisciplinary team.

Responsibilities

• Assess, plan, implement and evaluate packages of care for named patients.
• Support patients to be as independent as possible within functional limits.
• Work as part of a multidisciplinary team to ensure high standards of care.
• Support and deliver innovation within discharge practice and pathways.
• Collaborate with system partners to ensure seamless transfers of care.
• Be responsible for delivery and co-ordination of care for a named patient group.
• Work proactively to achieve unit and trust goals and promote policies and guidelines.
