Norfolk County Council is recruiting Reablement Assistant Practitioners for the Caring for Better Outcomes service in Norwich. The role supports people post-discharge, working with Occupational Therapists and local teams to restore independence and plan ongoing care where needed.

You will manage a countywide caseload, prioritise effectively, and contribute to a strengths-based Home First practice. A driving licence is essential for travel across the county.
