Job summary

Buckinghamshire Healthcare NHS Trust is seeking carers to join the Rapid Response & Intermediate Care team in Aylesbury. You will provide personal and rehabilitative care to adults in patients' homes, working closely with physiotherapists and occupational therapists to support independence and safe discharge from hospital.

A full UK driving licence and access to a car with business insurance for work purposes are essential.

Qualifications

Providing personal and rehabilitative care to adults in their own home environment.
Collaboration with Physiotherapists and Occupational Therapists to deliver care.
Willingness to work towards Level 3 Health and Social Care qualification in post.

Responsibilities

Providing personal and rehabilitative care to adults in their own home environment.
Being an integral member of the Rapid Response & Intermediate Care (RRIC) team, collaborating with registered Occupational and Physiotherapists to deliver rehabilitative, social and nursing care to patients.
Taking responsibility for delivering care, equipment, supervising exercises and other delegated tasks in patients' own homes.
Being responsible for the completion of delegated non-clinical tasks to support the efficient running of the team.
Improve the care of patients in the community, so patients can remain safely in their own home for as long as possible.
Have an innovative approach, excellent communication skills and willingness to work with other members of the community teams to ensure high quality service delivery.
Willingness to further develop your clinical expertise in a range of areas
