Job summary

Buckinghamshire Healthcare NHS Trust in Buckinghamshire is seeking an experienced Physiotherapist for a maternity cover within the Rapid Response, Intermediate Care and Community Physiotherapy team in Aylesbury. You’ll provide specialist triage, assessment and rehabilitation, and manage a community caseload to help patients stay at home and support early discharge.

The post requires HCPC/RCN registration, a physiotherapy degree, and at least two years post-qualification NHS/community experience.

Qualifications

• Registered Healthcare Professional with HCPC or RCN registration for Physiotherapist.
• Degree/Diploma in Physiotherapy or related field.
• Experience of post-qualification NHS community work or in-patient settings.

Responsibilities

• Provide triage, specialist assessment and rehabilitation for patients and carers.
• Plan and coordinate care to promote health gain and independence.
• Allocate workloads within the Rapid Response & Intermediate Care team.
• Collaborate with a multi-disciplinary team to ensure timely care delivery.
