Job summary

Buckinghamshire Healthcare NHS Trust is recruiting for a Band 6 Physiotherapist to join the Rapid Response, Intermediate Care and Community Physiotherapy service in Aylesbury on a part-time secondment basis.

You will triage and assess patients in the community, plan treatment programmes, and coordinate care with a multidisciplinary team to enable early discharge and home-based recovery.

Qualifications

• Registered Healthcare Professional with HCPC or RCN registration.
• Degree/Diploma in Physiotherapy, Occupational Therapy, Nursing, Paramedic Science.
• Experience of CPD (continuous professional development).
• Desirable: Electronic clinical systems

Responsibilities

• Provide triage, specialist assessment and evaluation of patients and carers.
• Plan care programmes to promote health gain and maximise independence.
• Allocate workloads within the RRIC team.
• Collaborate with multidisciplinary colleagues to ensure timely care delivery.
